Job Description:
Pay Rate $50.00/hr
Responsibilities/Duties:
- Attends patient/resident care conferences, team conferences, and rounds obtaining and reporting patient/resident information regarding patient's/resident's present level of functioning and future goals/plans in a clear and concise manner, as well as change in condition or treatment of patient/resident.
- Engages in timely, creative, and effective use of assistants, aides, and volunteers.
- Consistently displays a positive, caring, and responsive attitude towards employees and any individuals who interact with the facility.
- Demonstrates knowledge of, and rational for, physical therapy techniques; modalities; equipment use; and exercise programs.
- Establish both long- and short-term goals and a thorough treatment care plan by which the patient/resident can progress towards goals and modify care plan appropriately/timely when needed.
- If appropriate for patient/resident needs, collaborates with patient's/resident's families; significant others; and staff prior to patient/resident discharge in order to formulate an appropriate discharge or follow-up plan.
- When appropriate, effectively instructs patients/residents/families in techniques and home programs by structuring content/presentation to appropriately reflect patients'/residents'/family member's age, communication abilities, culture and educational level.
- Organizes schedule of patient/resident treatments taking into account the work demands of other employees/departments.
- Instructs patient/resident/family in the application, care, and precautions of orthosis/prosthetics as ordered by physician at Orthotic/Prosthetic Conference.
- Provides objective, accurate, and timely documentation according to policy on assigned patients/residents.
- Completes documentation for insurance requests in a timely fashion.
- Leaves a written, clear treatment plan (coverage notes) on all assigned patients/residents when scheduled out of the department.
- Demonstrates initiative to assist with the evaluation and development of documentation systems, forms, and charting mechanisms to facilitate efficient and effective documentation.
- Recommends equipment and/or supplies when inventory demonstrates the need to reorder.
- Attend one or more continuing educational programs and/or equivalent annually and provide staff in-service.
- Participate in program development/revision with the facility.
- Serve as a clinical instructor to supervise and teach a student affiliate following the facility clinical education manual and academic student manual.
- Actively participates in own performance appraisal evaluation and sets self-goals to accomplish during the following year.
- Maintains level of productivity as established for the department.
- Participates in department QA programs, including collecting of data, analysis, and documentation review.
- Assists supervisor in developing new innovative ideas for QA studies related to patient/resident care related problems and department operations.
- Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
- Performs all other duties as assigned by supervisor to ensure that the department functions and operates as smoothly and efficiently as possible.
Requirements:
Degree in Physical Therapy from an accredited program.
Staff: Basic entry level - experienced achieved through academic curriculum and clinicals and/or up to three (3) years work experience.
Possession of, or eligible for, a valid Physical Therapy license issued by the state of New York, CPR certification. If sitting for licensure examination, there is a condition of passing the licensuring exam within one year after employment and during the interim possess a New York State temporary permit.
